IMPORTANT INFORMATION FOR PASSPORT APPLICATIONS:

THE EMBASSY IS UNABLE TO ACCEPT APPLICATIONS FORMS THAT DO NOT INCLUDE ALL REQUIRED DOCUMENTS, OR APPLICATIONS THAT ARE INCORRECTLY COMPLETED. SUCH FORMS WILL RESULT IN DELAYS IN PASSPORT ISSUES. PLEASE HELP US TO HELP YOU BY TAKING TIME TO READ THE INSTRUCTIONS ON THE FORM.

All application forms are scanned to Canberra and new passports are returned to the Embassy within 10 WORKING DAYS.

It is very important that the application form be completed in BLACK PEN ONLY as the form needs to be scanned.

Applicants over the age of 18 applying for renewals of full validity passports can send in applications by post together with all relevant documentation. Applicants over the age of 18 applying for first passports, or renewal of limited validity passports, must apply in person. One parent must apply in person for applications for children's passports. Children do not have to attend the Embassy.

All applications for passports for children under 18 years of age, born outside of Australia, must also include the parents original marriage certificate and an official translation into English if necessary.

All required supporting documentation (birth certificates, marriage certificates, proof of change of name certificates etc) must be original documents and accompanied by official translations by Public Notaries if not already in English.

All applicants born in Australia who have changed their name in Israel or overseas must provide a name change certificate issued by the Registry of Births Deaths and Marriages in the State where they were born.

As of July 2008, applicants must also provide either their original full Australian birth certificate or their Australian cerificate of citizenship, along with their previous passport, when applying for new passports. If the certificate of citizenship does not include their place of birth and their sex, the applicant must also provide their original foreign birth certificate along with a notarized translation of the birth certificate if the certificate is not in English.

Additional means of identification showing name and signature and address are required for all adult applications. Please note the identification requirements on the application form.

IF PHOTOS DO NOT MEET THE REQUIREMENTS LISTED ON THE APPLICATION FORM, THEY WILL NOT BE ACCEPTED AND THERE WILL BE DELAYS IN THE PASSPORT PRODUCTION PROCESS. PASS22PORT PHOTOS ARE SCANNED AND MUST MEET THESE REQUIREMENTS.

The current passport fee (for October 2008) is AUD $208 (NIS 558) for a regular adult passport and AUD $104 (NIS 279) for a minor's passport. Seniors aged over 75 also have an option of obtaining a 5 year validity passport (AUD 104 - NIS 279). Fee may change at the end of each month. Please contact the Embassy to confirm the correct fee. Payment can only be made in Israeli shekels by cash or cheque. Fees must be lodged together with the application.

Corrections on the application form may be made with "Tippex" etc. Signatures cannot be corrected. SIGNATURES ONLY WITHIN BORDERS OF THE WHITE BOX


Information regarding Section 12 of the application form: Guarantor

Guarantor must be EITHER an Australian Citizen aged over 18, who has known the applicant for at least one year, or since birth, who is not related to the applicant and who possesses a current Australian passport with at least two years validity,
OR a person currently employed in one of the following occupations who is aged over 18, not related to the applicant and who has known the applicant for at least one year, or since birth:

Accountant Member of Chartered Institute of Company Secretaries
Barristers, Solicitors and Patent Attorney Pharmacist
Chartered Professional Engineer Police Officer with at least 5 years continuous service
Clerk of Court Postal Manager
Dentist Public Servant with at least 5 years continuous service
Doctor of Medicine Stipendiary Magistrate
Elected Representative of Parliament Teacher -full time with more than 5 years service at schools or tertiary institutions
Holder of Statutory Offices for which an annual salary is paid Registered Veterinary Surgeon
Judge Registered Nurse
Members of the Armed Forces with at least 5 years continuous service

Guarantors must endorse one passport photo as per the instructions.
