CONSENT FORM TO GRANT AN AUSTRALIAN VISA TO A CHILD UNDER THE AGE OF 18 YEARS - FORM 1229


While the information provided in the form requires only the non-travelling parent to complete the form, our local procedures require that both parents complete this form even if the child is travelling with one or both of them. 

 
The completed and signed form and certified copies (by a Notary / Notarion) of the following additional documents must be lodged with the child's visa application:

1.  Child's full birth certificate.  Copies of the Israeli IDs of the parents listing the child's details are not acceptable.

2.  Personal particulars / bio data pages of the passports of both parents and the pages containing the signatures of the parents.  

Important: Without the writen consent of both parents, the visa cannot be granted to the child. Exceptions are:

1.  The mother of the child is a single parent and the child's birth certificate does not state any details about the father of the chid; 

2.  The other parent is deceased. In this case please provide a certified copy of the death certificate of the parent;

3.  The parents are divorced and one of them has sole guardianship and custody of the child.  In this case please provide a certified copy of the divorce agreement, including the section relating to the child's custory and guardianship arrangements.

 4.  In all other cases, you must provide a court order confirming that a visa may be granted to child without the consent of one or both parents.
